The Oklahoma Tobacco Helpline wants to hear from YOU! If you or a loved one have quit smoking, take a 10-30 second video of yourself telling us about your achievements. We will be using the submitted videos to help inspire others to quit for their 2014 New Year's Resolutions. We are also going to send all video entrants meeting the above guidelines a t-shirt for your help!

OFFICIAL RULES:
1. HERE'S HOW IT WORKS: Entrants must be Oklahoma state residents and eighteen (18) years of age or older on date of entry to be eligible. Contest begins at 9:00am CST on 11/21/13 and ends at 11:59pm CST on 12/31/13. Entries can be made through the following method: Between 11/21/13 and 12/31/13 enter only via email. No purchase is necessary. Entrants may only qualify via the aforementioned method of entry.

2. ENTRIES: Only one (1) entry is allowed per person. Entries must be emailed to [email protected].

3. PRIZES: For each eligible entry a t-shirt will be awarded.

4. PRIZE REDEMPTION: Prize recipients will be notified via email by the 2nd week of January 2014. Entrants will receive a confirmation email requesting t-shirt size and mailing address.

5. ELIGIBILITY: Promotion open only to legal Oklahoma State residents who are 18 years of age or older, as of date of entry. Employees, officers and directors of The Oklahoma Tobacco Settlement Endowment Trust, VI Marketing and Branding, their immediate family members and/or those living in the same household of each are NOT ELIGIBLE. Void where prohibited by law. All federal, state and local laws and regulations apply. All federal, state and local taxes are the sole responsibility of Winner. Contest may only be entered in, or from, the state of Oklahoma. Entries originating from any other jurisdiction will be deemed ineligible. Tobacco free means nicotine free. Entries about quitting must be about quitting tobacco products including nicotine. Videos promoting alternative methods of quitting that are not FDA approved may not posted.

6. GENERAL RULES: By entering, entrant agrees to hold harmless and indemnify The Oklahoma Tobacco Settlement Endowment Trust, VI Marketing and Branding, and their respective parents, subsidiaries, affiliates, directors, officers, governors, employees and agents against any and all liability, damages or causes of action (however named or described), with respect to or arising out of (i) entrant's participation in the promotion, or (ii) the receipt or use of the Prize awarded herein. Acceptance of a Prize constitutes permission to use Winner's name and/or likeness and story for purposes of advertising and trade by The Oklahoma Tobacco Settlement Endowment Trust, VI Marketing and Branding and their respective parents, subsidiaries, affiliates directors, officers, governors, employees and agents, without further authorization, compensation, or remuneration, unless prohibited by law. By participating in this contest, entrants agree to be bound by these Official Rules and the decisions of the Sponsors. Sponsors not responsible for any typographical or other error in the printing of the offer, administration of the contest or in the announcement of Prize.
