This is the unedited recording feom one of tge most important courses @ASCRS 24 on therapeutic laser interventions on the cornea! I am presenting Through a complicated post LASIK case done elsewere, with rainbow 🌈glare then addressed also elsewere with retroflap ablation, presented to us in Athens with severe mesopic and scotopic symptoms despite 20/20 UDVA
We discuss-with this excellent group in Boston this year-some of the basics: correlating cornea diagnostics ( reflection topography, Pentacam, anterior segment OCT corneal thickness and epithelial maps. Use a RGP or Scleral contact to guide you if the symptomatology is corneal and then use the above data to design the ultimate therapeutic intervention-if there is one!
Other key points here is that we elected to go transepithelial PRK here ( outside the US available on the EX500 excimer by Alcon/Wavelight as the Streamlight procedure, not yet intergraded with Topo-Guided and Ray Tracing. Thus we used 2 treatment cards: a Raytracing for the low order aberrations ( slight myopia and astigmatism) the high order addressed by the Wavelight Plus raytracing software and the tilt! and a 2nd Streamlight card with just .50 Diopter of cylinder from the low order aberrations-obviously we deducted this from the RayTracing -Wavelight Plus
This was quite the rewarding case as we were able to document oblique coma and the corresponding total corneal power irregularity and then threat them off-label with the Ray tracing Alcon/Wavelight platform!
