In this video, Melissa and Dr. Hsieh discuss treatments for “uncomplicated” vs. “complicated” UTIs. There is, generally, a special emphasis on treating uncomplicated UTIs with antibiotics for no more than one week—if antibiotics are deemed necessary at all. However, as previously discussed, this particular therapy regime is not always effective and treatment for complicated UTIs is less resolved in the medical world. That being said, Dr. Hsieh has a couple of interesting solutions.

For patients who are experiencing severe UTI symptoms that have not subsided with standard antibiotic treatment, Dr. Hsieh has found that, for some, it is possible to combine antibiotics so that they might work synergistically with each other and, therefore, have a stronger effect. Another solution is to use intravesical antibiotics, which allows the antibiotics direct access to the urinary tissues. This second treatment also has less risk of side-effects resulting from antibiotic use, because the antibiotics used in such cases are known to not penetrate past the lining of the bladder.

Dr. Hsieh also clarifies what “antibiotic resistance” really means. Most importantly, it is the bacteria, not the person, that develops antibiotic resistance. Although, it is possible for certain individual physical characteristics to affect an antibiotic’s function and require longer exposure to an antibiotic than might otherwise be necessary, such as weight or the prostate.
