Alzheimer's disease is a gradual progression from normal to severe, and its diagnosis and treatment rates are currently low, often missing opportunities for early intervention. In the era of longevity, it has become an important issue to improve the quality of life of patients and ensure their autonomy to live well every day. It is crucial that multiple parties, including the community, hospitals and elderly care organisations, are involved to provide full-cycle care support. This includes brain health education, early identification and intervention, cognitive training, etc., and should start from childhood. When the condition progresses to the point where home care is difficult, professional care and end-of-life care can be provided by specialised elderly care institutions. Cognitive impairment-friendly community programmes have been launched in Shanghai and other places to build a full-process service system through screening, support, family empowerment and referral for professional intervention. In addition, elderly care institutions have set up specialised cognitive impairment care units that follow local standards, provide safe and appropriate care environments and services, emphasise a person-centred approach, and promote social interaction and quality of life for the elderly. Importantly, we need to recognise that cognitive disorders are not natural aging, and that the patient is far more important than the disease itself, and should be cared for in a holistic manner, respecting and caring for each and every one of them, so that they can feel warmth and dignity.
