How To Complete Medicare Form CMS 40B (Updated)

The Medicare Part B enrollment Form CMS-40B has been updated as of early 2024. This video shows you how to complete the form properly when you need to turn it in to your local Social Security office.

Starting in January 2025, the option to just "walk-in" and turn in Form 40B and Form L564 to your local SSA office is no longer available...unless it's a DIRE need situation.
