A very interesting case of Enucleation of the Periapical cyst in the upper anterior region with PRF placement.
This patient presented with Mild swelling on the palatal region.
Root canal was performed following which the lesion was planned for excision.
Radiographically it was well defined radiolucency involving the periapex of the lateral incisor extending up-to the canine.
Infection draining onto the labial side due to inclination of the tooth root , causing resorption of the labial cortex and perforation onto the lingual side causing palatal soft tissue swelling classically representing the shape of the "Dumbell", and hence the name Dumbell shaped swelling.
📌Points to Remember:-
1. Detailed history recording.
2. Clinical examination of the swelling
3. Radiographic examination of the swelling to examine the tooth involvement.
4. Plan of root canal treatment of the involved teeth before enucleation of the lesion.
5. Proper flap design depending on the lesion, semilunar or crevicular.
6. Trimming of the bony margins, apicectomy and cyst enucleation.
7. Irrigation of the defect.
8. Retrograde obturation if required.
9. Filling of the defect if required..with PRF or bone graft.
10. Proper alignment of the flap in place and suturing.
