I don’t know how I summoned him, but there are probably steps to having him appear in the fight.

Here’s what I did:
1:Chose agony mode
2:Killed that guy in the trash bag at the place Alex was left for dead, after speaking with Larry Davis
3:Did not kill Pedro, Did kill the dynamite Rando soldier
4:Went into Gardenias and fought the first rag maggot, all the pranksters, the shopping cart guy and his hairy friend, killed the Hammer king guy, and killed the telekinesis scissor guy
5:Finally get Georgie in my party, and gave the chalk to Augustus
6:Got the golden brolex and bought the cellophane bag (DIDN’T MEET OLLIE, OR GO TO THAT AREA AT ALL, DIDN’T DO CART RACES)
7:Killed Bob Versace
8:Talked to everyone Rando soldier and helped them. Gave the old man his son, have the fat guy the Rando rations, gave the mechanic guy a wrench, and gave the Rando bobble head to the Rando merchants
9:Killed Clingert and the big guy protecting the generator
10:Fought every boss (Will,Fishmen,Mother maggot)
11:Killed the creep and the crow guy after giving him 1 old crow
12:Killed all the rag maggots on the way to Roland

If you do all this then I think it’ll get Fly to appear during the boss fight.
