Due to the proximity of the sacroiliac joint to the hip and the spine, pain of SI joint origin may be attributed to a hip or spine condition rather than the sacroiliac joint itself. SI joint pain is an unappreciated problem causing low back pain . Clinical and radiological evaluation is not usually reliable. The only reliable method for diagnosing SI joint pain is by injection.


Symptoms of sacroiliac joint dysfunction include:
•Lower back, buttock, back of thigh and knee pain.
•Occasional groin pain
•Difficulty and discomfort while sitting.
•Patient frequently changes position to become comfortable.
Tests used to determine the presence of SI joint pain:
•Finger test
•Faber test
•Straight leg test
Differential diagnosis of SI joint pain
1-Trochanteric bursitis
2-Piriformis syndrome
3-Myofascial pain
4-Lumbosacral disc herniation and bulge
5-Lumbosacral facet syndrome
6-Lumbar radiculopathy
7-Cluneal nerve entrapment.
Cause of SI joint pain
1-Leg length discrepancy
2-Mechanical dysfunction
3-SI joint infection
4-Ankylosing spondylitis
5-Crystal arthropathy
6-Pyogenic arthropathy
7-Post spinal fusion
8-Stress fractures of the sacrum.
Treatment of SI joint pain
•Anti-inflammatory medication
•Physical activity, manipulation, massage, yoga, exercise and acupuncture.
•SI joint injections; steroids, platelet rich plasma, prolotherapy.
•Radiofrequency ablation.
•Stabilization or fusion of the SI joint
