Hi, I’m Dr Jay Mehta, and in this video, I’ll explain the role of BCL6 in the endometrium.
BCL6 stands for B-cell lymphoma type 6 marker. It’s a crucial diagnostic tool that helps us assess conditions like endometriosis, endometritis, and chronic pelvic pain. All three are major contributors to failed implantation, unexplained infertility, and long-standing pelvic discomfort.

BCL6 in Infertility: Detecting Endometritis, Adenomyosis & Implantation Failure! | Dr Jay Mehta
What Does BCL6 Tell Us About Your Endometrium?
This marker reflects how resistant your endometrium is to progesterone. A normal, healthy endometrial lining responds well to progesterone in the second half of the menstrual cycle. But when there’s resistance, this process gets disrupted—and BCL6 gets upregulated. That means if we detect high BCL6 levels, we often suspect silent endometriosis or inflammation.
Can It Detect Endometriosis or Endometritis?
Let me clarify—BCL6 doesn't grade endometriosis, but it can strongly indicate its presence. In the case of endometritis, it detects inflammation when used along with other immune markers. So, if someone has chronic pelvic pain or failed IVF cycles, this test becomes a powerful tool in uncovering hidden causes.
Why Is This Test Necessary in Early Adenomyosis Cases?
Sometimes, adenomyosis sits deep within the posterior uterine wall. It’s small and doesn’t show up clearly on regular ultrasounds. That’s where BCL6 helps. A raised marker in such patients often helps us catch these subtle lesions before they cause major issues in fertility.
How Does BCL6 Fit Into a Full Immune Assessment?
We never use this marker in isolation. In my clinic, we combine BCL6 with other markers like uterine natural killer cells (uNK), M1 macrophage indicators, and interleukin-8. We also study your high-resolution ultrasound findings to build a complete immune and inflammatory profile. That helps us decide whether medication, immune modulation, or surgical correction will work best.
What About Recurrent IVF Failures and Miscarriages?
If you’ve had three or more abortions or two failed IVF cycles despite having good embryos, please don’t ignore it. These are classic signs that something deeper is going wrong in the endometrium. BCL6 can be the missing link. With a proper immunological evaluation, we can identify hidden conditions like progesterone resistance, endometritis, or silent endometriosis—all of which could be treated effectively.
How BCL6 Helps Diagnose Silent Endometriosis?
This marker becomes especially useful in patients who suffer from chronic pelvic pain, but their scans and regular tests show nothing abnormal. That’s what we call silent endometriosis. We use BCL6 testing through endometrial biopsy in such cases, and in many, we uncover hidden endometriotic patches in areas like the pararectal or paravesical spaces. Once surgically treated, these patients often get complete pain relief.
