You can't make a fatality in the original game as there are none. This is a ROM patch. That's the only way you can have this.

Don - “Turtle Power” - Down, Down, Down [Far]

Hothead - “zu Asche” - Forward, Back, Punch [Close]
Shredder - “Sayonara” - Forward, Back, Punch [Close]

To apply the patch to the ROM you need a program like Lunar IPS and such. Make a copy of the ROM rename it to something else and apply patch to it. This way you will not ruin the original ROM.
