A fistula in ano is an abnormal communication between the anal canal and the skin around the anus. While simple fistulas are usually single-tract and easier to treat, a *complex fistula* is more complicated because it may involve multiple tracts, cross through important sphincter muscles, or be associated with chronic diseases such as Crohn’s disease, tuberculosis, or previous failed surgeries.

A complex fistula is defined by one or more of the following characteristics:
Multiple branching tracts
Involvement of more than 30–40% of the anal sphincter
Horseshoe extensions around the anus
Recurrent fistulas after previous surgery
Association with chronic infection, radiation, or systemic disease

Patients often report:
Persistent pus or fluid discharge around the anus
Pain and swelling that worsens on sitting or passing stool
Irritation and itching around the perianal region
Multiple openings around the anal skin
Recurrent perianal abscesses
History of failed operations or repeated infection

On clinical examination, doctors may notice:
More than one external opening
Induration and fibrosis in the perianal skin
Scars from previous surgeries
Discharge points in unusual locations away from the anus

Complex fistulas can severely impact the quality of life. They are associated with constant discomfort, embarrassment from foul-smelling discharge, and repeated abscess formation. Delay in proper treatment can lead to damage of anal sphincter muscles and even incontinence. Understanding symptoms early helps patients seek timely surgical advice.
