Abnormal bleeding is one of the most common reasons why women see their gynecologist. The PALM-COEIN system details the causes of abnormal bleeding.

PALM-COEIN classification system (FIGO)
i. Polyp
ii. Adenomyosis
iii. Leiomyoma (fibroid)
iv. Malignancy (cancer)
v. Coagulopathy
vi. Ovulatory dysfunction
vii. Endometrial
viii. Iatrogenic
ix. Not yet classified (unknows)

Polyps look like fleshy tissue that can be benign, precancerous or cancerous.
a. Polyps can be diagnosed on transvaginal ultrasound, pelvic MRI, sonohysterogram or hysteroscopy
b. Approximately 56% of polyps are seen on transvaginal ultrasound
c. Sonohysterography shows polyps better than ultrasound

d. Endometrial biopsy will sometimes show a fragment of a polyp
e. If you have a polyp the treatment is usually removal
f. Usually after the polyp is removed most women will have a resolution of their abnormal bleeding
g. If the polyp has cancer or precancer changes like uterine hyperplasia your doctor may recommend additional surgery
