Atrial fibrillation is a problem cardiac surgeons have been dealing with since the 1980's. It was Jim Cox who pioneered the field and inspired generations of heart surgeons to perform the Cox Maze procedure. With the advent of minimally invasive techniques and the insight from Electrophysiology the strategies have changed. We no longer open the sternum and use cardio pulmonary bypass to cut the heart into pieces and remove the left atrial appendage. We now use thoracoscopic minimally invasive techniques to apply radiofrequency energy and obtain very effective pulmonary vein isolation and use a new device to close the left atrial appendage. Hence providing long term benefit for all patients. Hybrid approaches provide for even better results and it is through a collaborative effort that the best results are obtained.
