Sanctum Minor Boons tierlist:

S tier: scrying orb, sanguine vial, ornate dagger, hare's foot, gilded lyre
A tier: ununtuned lute, tarnished descry, silver chalice, rusted chimes, mellifluous chorus, fountain of youth, glowing orb, enchanted urn (only if you have relics), adrenaline vial
B tier: wooden effigy, prayer beads, lustrous pearl, gold trophy, fright mask, divinia's gift, black pearl
C tier: engraved orb, tarnished coin, silver tongue, mirror of fortune (could be S tier if you already have divines but generally it comes up too early) , gold magnet, gold mine, rusted descry
D tier: viscous ichor, assassin's blade.

Best Major Boons:
All seeing eye, crystal chalice, holy water, dekhara's necklace, the boon that makes you immume to all damage (if you grab it before the boss room)

Sanctum Afflicitions tier list:
Run ending: gold smoke, accursed prism, black smoke, deceptive mirror
Avoid without good reason: spiked shell, blunt sword, worn sandals, unholy urn (only if you have relics) , purple smoke (nearly run ending if you get at start), rapid quicksand, dark pit
Don't pick if other options: poisoned water, liquid cowardice, honed claws, voodoo doll, empty trove, mark of terror
Mildly inconvenient: split purse, weakened flesh, spiked exit (only up here because people think this afflicition is a lot worse than it actually is. its based off your current, not max resolve. ) floor tax, door tax. ( i wouldnt combine too many of the gold loss afflictions because it can be a pain retrieving some rewards if you doo too many)
Free: red smoke, unquenched thirst, chains of binding, unallowed ring, rusted coin, hungry fangs, gargoyle totems, concealed anomoly, hemmorage, tatterted blindfold, charred coin.
