Medicare pays for Transitional Care Management (TCM) for a 30-day period after a patient’s discharge from a facility. This benefit supports patients as they transition from an inpatient facility back into the community.

This session will cover:

• Eligible discharge facility types
• Eligible services including face-to-face and non-face-to-face care
• Billing requirements

This session is intended for newly enrolled Medicare providers that will bill Medicare using the CMS 1500/837P transactions.
