Before we go further,
I want to share the mindset behind this channel.

I am not here to teach you a single injection,
a specific syndrome,
or a list of structures to memorize.

I am not a surgeon,
and I am not focused on medication-based treatment either.

I am a pain interventionist.

For me, intervention does not start with a needle.
It starts with understanding pain.

Pain is not a diagnosis.
Pain is a biological and dynamic process.

If we do not understand
why the pain started,
why it has this intensity,
and why it has lasted this long,
any intervention becomes guesswork.

That is why, in this channel,
I spend so much time talking about
pain mechanisms,
dynamic factors,
and pain prerequisites.

Some may feel
that I describe too many pain generators,
or that the explanations are too long.

But this is intentional.

The gluteal region, especially in sitting-related pain,
is one of the most complex convergence zones in the body.
Spine-related pain, pelvic mechanics, neural irritation,
tendon pathology, and bursal inflammation
can all overlap in the same area.

If we look from only one perspective,
everything starts to look like the same disease.

This channel is not about choosing sides
between spine-based thinking
or MSK-based thinking.

My goal is a neutral, integrated view,
where pain is read first,
and structure is interpreted later.

I want you to develop the ability
to listen to pain itself.

Pain nature.
Dynamic behavior.
Pain intensity.
Pain duration.

These alone already tell us much more
than many imaging findings.

That is why I often say:
the patient interview is not a preliminary step.
It is the core diagnostic process.

If you can read pain correctly,
intervention becomes precise.
If you cannot,
even the best technique may fail.

for one clear purpose.
