Basically hemorrhoids are nothing but bunch of blood vessels arranged in three groups inside the anal canal and partly outside the anal canal. Inside ones are called internal hemorrhoids and outside ones are called external hemorrhoids when they are enlarged. Normally when we refer to hemorrhoids or piles most of the times it is internal hemorrhoids, in internal hemorrhoids we have got 4 different stages, 1 st stage is first degree piles there the hemorrhoids are enlarged to an extent but they are not coming out, so nothing is seen from outside but occasionally they may have bleeding when they pass motion. Second degree of hemorrhoids a small protrusion comes out when they apply pressure and goes out spontaneously, in third degree hemorrhoids it comes out it has to be pushed out or slowly it goes back. Fourth degree hemorrhoids they remain always outside,. In external hemorrhoids they will not even notice that they have hemorrhoids because they are soft structures, but when blood clots inside they cause painful swelling that is how they are noticed. So these are types of hemorrhoids. Apart from that hemorrhoids can also be caused secondary to some other disease like rectal cancer where the pressure will increase the flow into the blood vessels, so that should not be missed when ever we are examining the patient with hemorrhoids.
