Gall stone pain is mostly in the right hypochondric area, that is right upper abdomen. So if gallstone is persistent, dull aching pain and it is localized to one area and it may even radiate to right shoulder and gallstone if it is causing block and obstruction, then patient may have high fever and palpable swelling in the upper abdomen, whereas appendicitis pain may start initially in the upper umbilical area and later it radiates to left fossa. So when you touch yourself you may have pain in the right or left fossa when there is appendicitis. In gallstone pain it is very dull aching whereas in appendicitis pain it may be a colicky pain or it can b a dull aching pain if there is a stone in the appendicular area, and if it perforates there is a severe pain all over the abdomen which can happen with perforation of the gall bladder also.
