For over 50 years, the Ombudsman Services program of Contra Costa, Solano and Alameda has provided fierce high-quality advocacy on behalf of residents and their families in long-term care. Since then, the organization has expanded its services and scope of work, reaching a movement of vulnerable older and disabled adults across the nation. Work that wouldn’t be made possible without you. That’s why we’re excited to share with you the news of our new name. Say hello to Empowered Aging.

This new name captures how far we’ve come, who we are today, and positions us for a successful new future as we continue to support and strengthen the journey in aging. Empowered Aging enables us to connect with a broader audience as we expand our reach and services even further. Healthcare Career Pathway is a collaborative partnership in Contra Costa County with Empowered Aging, Opportunity Junction, Mt. Diablo Adult Education, and several skilled nursing facility partners that addresses the critical shortage of qualified healthcare workers. We offer a unique program that removes obstacles to success. Our goal is to help you graduate with the certification you need to become a caregiver our community needs. Long-Term Care Ombudsman
As long-term care ombudsman, we are expressed wish advocates for residents in long-term care facilities. We are the only program mandated by law to make regular unannounced visits to facilities, ensuring that residents live free from abuse, neglect, and receive quality care. Master of Social Work Internship
With a mission to ignite the interest of emerging social workers in a career working with older adults, Empowered Aging has designed an MSW internship program that provides full training and hands-on experience advocating for older adults and adults with disabilities. Resident Empowerment Program
More than 150,000 older adults are living in Contra Costa County. Many will need long-term care to help them maintain health and wellness. The Resident Empowerment Program (REP) seeks to ensure that these residents’ intentions for aging and end of life are respected by the protection of necessary legal documents such as Advance Health Care Directives and Power of Attorney and established relationships with resident advocates and legal representatives.

A first-of-its-kind partnership, Contra Costa Legal Services and Empowered Aging have come together to provide innovative and responsive screening, education, and legal counsel to residents of Residential Care Facilities for the Elderly (RCFE).
