This video shows the appropriate way to carve rib cartilage for a revision rhinoplasty. While this typically takes some time, it has been edited and sped up. Rib cartilage is often used in revision rhinoplasty since the patient's own nasal cartilage is inadequate or insufficient. Ear cartilage can also be used but when structure is needed to rebuild the nose either by building up the bridge to correct nostril notching, loss of tip support, projection, and/or all of the above, rib cartilage is the best option since it has unparalleled structural integrity and strength. One problem with rib cartilage is the potential for warping and for that reason, Dr. Williams uses what is referred to as concentric carving so that only the center part of the rib cartilage is used for structural rebuilding. In other words, the outer aspects of the rib tend to warp initially and sometimes over long periods of time. Harvesting the rib is taking lower on the chest wall towards the sternum where the rib turns into cartilage and serves as a very useful source. Meticulous carving allows an experienced rhinoplasty surgeon to create very precise grafts that can be used in the rebuilding process.
