Structural Heart Intervention Series: Challenge Valve in Ring TMVR (Sachin Goel, MD)

This talk features a case of valve in ring (ViR) transcatheter mitral valve replacement (TMVR) in a patient with prior surgical mitral repair, now presenting with symptomatic severe mitral stenosis. Based on CT derived measurements, the patient was at high risk for left ventricular outflow tract (LVOT) obstruction with TMVR. The procedure was performed with adjunctive tip to base LAMPOON (Intentional laceration of the anterior mitral leaflet to prevent left ventricular outflow tract obstruction during transcatheter mitral valve replacement) technique. Dr Goel discusses CT assessment of risk of LVOT obstruction with TMVR, and the various types of LAMPOON procedure to reduce risk of LVOT obstruction with TMVR.

Objectives:
1. To understand the risk of LVOT obstruction with TMVR based on CT measurements.
2. To understand the electrosurgical catheter-based LAMPOON technique to reduce risk of LVOT obstruction with TMVR.
3. Review a case of symptomatic severe mitral stenosis in a high surgical risk patient with prior mitral valve repair, treated successfully with tip to base LAMPOON assisted ViR TMVR.
