In a urine sample examined under high magnification (high power field, or HPF), the presence of pus cells (white blood cells) and red blood cells (RBCs) can indicate various conditions, primarily infections and inflammation within the urinary tract. Normal urine contains few to none of these cells, and higher counts suggest a potential problem.
Pus Cells (White Blood Cells):
Normal Range:
In healthy adults, the normal range for pus cells (also known as pyuria) is typically 0-5 per HPF.
Elevated Levels:
Counts exceeding this range, especially 10-12/HPF or higher, are generally considered abnormal and can indicate a urinary tract infection (UTI) or other inflammatory processes.
Interpretation:
Higher pus cell counts can suggest bacterial or other infections in the urinary tract, such as cystitis (bladder infection) or pyelonephritis (kidney infection).
Other Causes:
While UTIs are the most common cause, pus cells can also be elevated due to other factors like sexually transmitted diseases, certain medications, or other medical conditions.
Red Blood Cells (RBCs):
Normal Range:
Similar to pus cells, RBCs should also be minimal in a healthy urine sample. A few RBCs may be present, but significant increases can be a sign of various issues.
Interpretation:
The presence of RBCs (hematuria) in urine can be caused by a wide range of issues, including:
UTIs: Infections can cause irritation and bleeding in the urinary tract.
Kidney stones: Stones can irritate and damage the lining of the urinary tract.
Glomerular diseases: Issues with the kidney's filtering units can lead to blood in the urine.
Other conditions: Tumors, trauma, or certain medications can also cause hematuria.
RBC Casts:
The presence of RBCs within casts (cylindrical structures) in the urine sediment is particularly significant. It often indicates glomerular disease (kidney disease).
