The goal during the first step of treatment is for the client to learn that doubts are behind the obsessions and responsible for most of the OCD symptoms. In the majority of cases, the entire obsessional preoccupation of the client can be simplified tremendously by tracing it back to a few single doubts, and it is important to convey this notion to the client. In other words, as overwhelming, detailed and intricate as the OCD may seem to the client, there usually are only a few doubts by which most of the obsessional preoccupation stands or falls. A complete understanding that a doubt is the central problem also helps the client to lose some of their focus on the consequences and compulsive actions of the doubt.
