The PACT (Prevention and Access to Care and Treatment) project serves chronically ill patients in the most impoverished and marginalized neighborhoods of Boston. For these patients, PACT is their last hope to regain their health and their lives. PACT is a project of Partners In Health and Brigham and Women's Hospital.

In treating HIV/AIDS and other chronic diseases, the ability to take your medication each and every day or stay on a diet is key. It sounds simple...but for those struggling against poverty, mental health issues, and addiction, what seems like a simple task is an enormous challenge. Sharing the community-based approach of Partners In Health, PACT community health workers are trained to serve their patients by working with them daily and in person to understand and develop medication adherence practice. They help disadvantaged patients keep doctor's appointments and navigate the complex U.S. health care and social service systems. This model of care has been shown to both save lives of HIV patients and save money for the health care system, by, among other things, improving patient adherence to complex medication schedules, and decreasing costly visits to the ER.
