Cleft lip & palate is a very common problem. It has happened all over the world, in developed countries, in developing countries. It has got a very high incidence in our country also. But this is one of those fortunate problems which can be treated fully. Children with cleft lip and palate can lead a completely normal life. However, to have the best results you should get the surgery done at appropriate time. The timing for cleft lip surgery if the child is born with complete cleft lip and palate unilateral or bilateral is within the age of 3 months. The surgeon will also close the anterior half, i.e., the front half of the palate. Then at the age of 9 months, when the speech begins then you can go in for the cleft palate surgery. This should be followed by speech therapy. Now at this time, at the time of cleft palate, if we find that the Eustachian tube is blocked, we will suggest a grommet insertion. Grommet insertion will improve the hearing of the child and improves speech also. Now once this is done, the child is allowed to grow, child goes to school normally. If the child has complete cleft lip and palate the child will require repair of the alveolar cleft. Alveolar cleft is the cleft between the teeth. That cleft needs to be repaired at 9-11 years. What this does is to allow the teeth to come out normally. The child also has to go for orthodontic treatment at this point. At once the child reaches the adulthood, 16-18 years, there may be a requirement for rhinoplasty or orthognathic surgery. Rhinoplasty is the correction of nose and at the same time if any scar revision is to be done can be done. When nose is corrected your plastic surgeon will tell you if a bone graft needs to be put. In some people, if the jaw is extremely receded or is underdeveloped then orthognathic surgery in terms of sagittal split osteotomy is done to slide the jaw upwards or backwards or a maxillary advancement procedure will be suggested. So these are the various surgeries that a child with cleft lip and palate has to undergo.
