So priapism is a condition where there is continuous erection of the penis for more than 4 hours without any stimulation. So the symptoms coming to priapism usually patient may have continuous erections without any stimulation and there are various causes of priapism. It can be venous or it can be arterial. So usually patient with priapism they may have predisposing factors like sickle cell anemia or they can be trauma to the penis or pelvic area. So these causes which can lead to priapism patient usually present to emergency saying that his penis is erect for the last 4 hours or more than 4 hours and he has pain in the penis and pelvic area. So clinically whenever the patient based on his history we will clinically examine him and we'll do few investigations to rule out the priapism the thing is initially we do blood gas analysis of the blood aspirated from the penis to look for what is the condition or what is the type of blood gas analysis. Most commonly it will be increased carbon dioxide that is acidotic type of blood gas analysis and the next patient will have severe pain that is continuous pain which is dragging throughout his penis and pelvis area. In addition to this we usually advise for Doppler study of the penis region to look for what is the cause after diagnosing the patient with priapism if you are not treating the condition of priapism or if there is any delay treatment in the priapism patient may land in erectile dysfunction which can be permanent. So the initial treatment what we do whenever patient present to emergency with priapism is we usually aspirate the blood from the corpora of the penis. So it will give immediate relief but there are chances that even during aspiration patient may not get relief of his symptoms. Then we give additional supplementation of drugs or we need to do surgical procedures like some procedures which can be done either in the emergency department or some patients may need to be taken to the operation treatment for the cure of this disease.
