This presentation discusses evaluation and management (E/M) codes for outpatients, codes 99202-99215.

These outpatient codes use time spent with the patient of medical decision making (MDM) to select the correct code.

Medical decision making (MDM) may be at these levels: Straightforward, low complexity, moderate complexity and high complexity.
The levels are determined by three factors: number and complexity of problems addressed; amount and/or complexity of data to be reviewed and analyzed; and risk of complications and/or morbidity or mortality of patient management.
The factors for number/complexity of problems include Category 1 (order/review of tests); Category 2 (interpretation of tests); and Category 3 (discussion with another provider).

The correct code is selected using either medical decision making or time spent providing the service.
