Up to 85% of people with spinal cord injury (SCI) will have some form of a condition called 'neurogenic bladder' - a disruption of the ability to store and void urine. Bladder dysfunction is among the leading causes of re-hospitalization after SCI.

What is the 'decision to treat' based on in people with SCI?
People with SCI usually present with Urinary Tract Infections (UTIs) with symptoms resembling the flu (chills, fever) or they may depend on the malodor of their urine (due to lack of sensation). So, the decision to treat is usually based on these symptoms and the results of the urinalysis.

How do you get a urine sample from your patient with SCI?
Taking a urine sample directly from a leg bag or from someone who has had a catheter in for any length of time will be misleading and likely have loads of different bacteria present. Getting a 'clean catch' from the urethra will help diagnose the type of bacteria present much better.

What are some of the best things that Family Doctors can do for their patients with SCI?
Focusing on the bladder, ensuring that they have had a referral to Urology. Bladder in SCI is complicated and is often changing, so seeing a urologist regularly is recommended. They may form stones more readily, they usually get UTIs more regularly, so looking at things like changes in creatinine levels over time is usually a good thing.

Can people with tetraplegia regain bladder function?
There is a prognostication for people with incomplete tetraplegia; a large multi-center study in Europe found that lower extremity motor score (LEMS) and manual muscle testing scores (mainly 4s and 5s) have found that some people regained bladder function.
