The Hospital at Home Users Group [hahusersgroup.org], in partnership with the American Academy of Home Care Medicine [https://www.aahcm.org] and the American Hospital Association [https://www.aha.org], is pleased to present a webinar series for hospital and system leaders interested in learning about implementation opportunities and challenges associated with the new CMS waiver supporting Acute Hospital Care at Home.

Hospital at home can be a good option for lots of patients, but it is not right for everyone. This webinar will address how to ensure that hospital at home programs find and attract patients with the appropriate acute conditions and living situations that lead to robust census levels and generate the best possible outcomes. It will discuss approaches to identifying patients in various settings (e.g., in the emergency department, from floors, in the clinic and community) and the challenges and opportunities associated with these efforts. Building from the experience of two leading hospital at home programs, this webinar will explore the full range of patient policies and procedures that together serve as a core element of a successful hospital at home program.
