Objective: In this era of big data, integrating data from different sources is essential for secondary data analyses. Heterogeneous data , however, are usually represented in terms of various terminologies. Thus, it is important to link them through terminology mapping. The Medical Dictionary for Regulatory Activities (MedDRA) and International Classification of Diseases (ICD) are two commonly used medical terminologies. Our main objective is t o provide a quantitative and qualitative analysis of the mapping status between MedDRA and ICD.
Materials and methods: We focus on evaluating the current mapping status through the Unified Medical Language System (UMLS) and Observational Health Data Sciences and Informatics (OHDSI). For a mapped pair, we summarize the current mapping statistics and further evaluate the quality of the current MedDRA - ICD mapping; for unmapped terms, we use our self - developed algorithm to rank the best possible candidate s and evaluate the semantic similarity.
Results : The mapped pairs of MedDRA - ICD terms that were identified from UMLS and OHDSI cover 27.23% of the overall MedDRA preferred terms (PT). The systematic quality analysis demonstrated that, among the mapped pair s provided by UMLS, only 51.44% are considered an exact match. For the 2,400 sampled unmapped terms, 56 of the 2,400 PT terms could have exact match terms from ICD. Among potential candidates for additional partial mappings, we also identified 44.95% of the broader, narrower relationships that were also found in the UMLS mapping.
Conclusion: Some of the mapped pairs between MedDRA and ICD are not exact matches due to differences in granularity and focus. For 72% of the unmapped PT terms, the identified ex act match pairs illustrate the possibility of identifying more mapping pairs. Referring to its own mapping standard, some of the unmapped terms should qualify for the expansion of MedDRA to ICD mapping in UMLS.
