Some of the most vulnerable patients are being sent home from hospital too soon - feeling afraid and with very little support. Those are the findings of an independent report into hospital discharges. It's revealed there's been a sharp increase in the number of complaints. Cases include frail and elderly patients being sent home with no one to care for them. Our Health Correspondent Catherine Jones reports.
