For almost 30 years, the Alfredson protocol has been used to manage non-insertional Achilles injuries. Unfortunately, the long-term outcomes using this protocol are not great, as 60% of people treated with this exercise routine report pain 5 years later. This video reviews new research showing that exercises that improve intratendinous fluid flow dynamics and/or increase the degree in which tendon fascicles slide over one another stimulate tendon remodeling by activating tenocytes. The exercises are done on the ToePro exercise platform as it is an easy way to target synergists, which can offload the Achilles tendon as it heals. Strengthening flexor hallucis longus is especially important.
