Probably both.

This guy is cool as fuck if you ask me. I prefer the first theory of him being a manifestation of mad cow disease. Especially with his appearance of broken legs and the death screen, referencing the deterioration of the brain. I especially love this dooming chase theme. It's such a heavy sense of dread and honestly freaked me the hell out the first time I heard it in-game.

Good monster overall. Maybe I'm just a sucker for mental shit in horror, who knows?
