Krishna says whatever one thinks during the time of their death has special significance. In their next life, they will attain whatever they think of during the time of death in this life.
Supporting story from Bhagavata 5th Skanda - Bharata Chakravarty (born as a doe for thinking of a doe during his death, born as a human for thinking of his human form in his doe life).
A man obsessed with money might think of money during death. He will be born as a wealthy man in his next life. Similarly a man who is obsessed with Bhagavanta attains Moksha.
Sada sadbhaava bhaavita - If one's mind is constantly focused on the thing, only then is it possible to remember it at the end of life. This one must try their whole life to focus constantly on Bhagavanta. Even then it might not be possible, unless it's the final life (Charama Shareera).
Story from readers digest of a girl who was unable to marry her lover, committed suicide and was reborn as a crocodile, that then falls ill. Her lover finds the crocodile and nurses her. The girl might have thought of animals in the water while dying (she fell into water to die).
Bheeshma was unable to see Krishna at the end of his life despite trying. It was not his Charama Shareera. He saw the event that affected him the most - Krishna bleeding from his arrow. He was stricken with repentance.
Negative emotions like hatred, anger, revenge really make a strong impact and one may remember them more during death. Thus, one must try to remain away from these.

Krishna says - thus, one must always think of Bhagavanta. Even during fighting in a war, one must think of Bhagavanta.
One can think that Bhagavnata is the one making them fight. This Anusandhaana must be in everything.
Bhagavanta is the one making us do bad deeds as well, not just good deeds. Sajjanas, being aware of this, even offer their mistakes to Bhagavanta and ask forgiveness. Thus, bad deeds don't affect Sajjanas as much as they affect Durjanas.
It is a misconception that Bhagavanta only makes us do good deeds.
He is the one controlling us in all Avasthas - Wakefulness, dream and deep sleep.
There is no need to wait for the right time or clothes to think of Bhagavanta. It should be constant. Shlokas like "Tad Eva Lagnam" and "Yasmaret Pundareekaaksham" show that His remembrance itself brings purity.
Mahabharata has the story of Dharma Vyaadha, a hunter who sold meat but was a greater Saadhaka than Rishis. He was very sincere and sold meat just to take care of his family, without any greed and attempt to accumulate wealth.
Kautilya's Artha Shastra explains how a merchant should price his merchandise in accordance with Dharma. It takes into account price at which the merchant buys the product, transportation, salary for employees, household expenses, and emergency buffer. Anything more than this is greed. Dharma Vyaadha was very sincere this way.
Veda Vyasa's writings have many characters who show this Bhaava Shuddhi - purity of mind. Another example is from Mahabharata - a scholar who is out of a job tries to comit suicide. A fox confronts him and tells him that it doesn't have hands to eat or chase away flies but he still continues to live. A human has hands to work and achieve things but still wants to die. The scholar asks how the fox knows this and it says it was a scholar in the past life but arrogance brought him here.
Mayy Arpita Mano-Buddhih - “Offering/focusing both mind and intellect to Me” - One must offer both mind and intellect to him. The mind's function is doubt (Sankalpa Vikalpaatmakam Manah) while the intellect’s function is decision (Nirdhaara). Even when the mind wants to waver as is its function, if the intellect is firm, one will remain focussed on Bhagavanta.
If the mind wavers, intellect can hold it firm. If the intellect itself isn't firm (deep doubts), then it becomes conviction and it is very hard to overcome.

Krishna says that through constant practice, one must control Chitta (memory) to only remember those memories of topics of Bhagavanta and keep everything else out.
Both Manas and Chitta can be controlled by constantly feeding it things that you want it to remember. If you experience a lot of material things, the mind will ask for more of it and the memory will remember these thoughts more. Thus, one must feed these Bhagavad Vishayas and they will think more of Bhagavanta itself.
