So, this scene is called "Dweller in the Dark". Sadly, I wasn't able to record the after boss fight scene, but this is what happens:

Protagonist defeats final boss, attempts to absorb the relics, Io stops him/her, she absorbs them as a "thank you" to the protagonist, sacrificing herself in the process, turning into a giant ffxiii-like tree. One bead (the one shown in this cutscene) falls down to the floor, representing Io's strength. A couple of years went by, and everyone (including Karen, Nicola, Emily, Eva and Aurora) are alive and well at the home base. Then, this cutscene follows, heavily implying a Code Vein 2/Maybe a God Eater crossover.

I think that the only thing you have to do to get this ending is save everyone/restore the 4 relic character's memories. Also, pretty sure this is the true ending.
