What Is an Ileostomy?
An ileostomy is a temporary opening we create during rectal cancer surgery. We bring the last part of the small intestine—called the ileum—onto the surface of your belly. This allows digested food to exit directly from this opening, bypassing the large intestine so it can rest and heal.
Why Do We Use It?
We often use a temporary ileostomy to support healing after rectal cancer surgery. It’s important to understand how to manage it properly once you’re discharged, because it affects your daily routine and care.
Learning the Basics of Stoma Bag Management
Right after surgery, we start involving the patient and their family in ileostomy care. We teach them how to cut, prepare, and fix the stoma bag properly.
If you’re new to this, it’s important to understand what’s in your stoma kit, how to use stoma powder, stoma cream, and how to maintain skin hygiene around the stoma. We’ve created separate videos on stoma bag cutting and application—those will help you learn step-by-step.
One-Piece vs Two-Piece Stoma Bags
You should also know the difference between a one-piece and a two-piece stoma bag. We usually prefer a one-piece system right after surgery because it’s easier to apply and remove. It’s also more affordable, and useful when you’re still learning or need to change the bag frequently due to leakage.
Understanding and Managing Stoma Output
In the beginning, ileostomy output is liquid and sometimes greenish. That’s because bile acids are not getting absorbed properly anymore—they’re being expelled directly.
This output will gradually become paste-like or semi-solid over the next 4 to 8 weeks as your intestine adjusts. Be patient. This is completely normal.
Hydration and Electrolyte Care
While your output is still liquid, you lose a lot of fluids—sometimes 1.5 to 2 litres daily. If you don’t stay hydrated, you risk dehydration and electrolyte imbalance. This can cause fatigue, low sodium, low potassium, or even weight loss.
So, don’t just drink plain water. Prepare an ORS or electrolyte solution at home and sip it throughout the day. Coconut water, lemon juice, or fruit juices also help, but always consult your doctor if you have kidney issues, diabetes, or blood pressure problems.
