“Medication reconciliation is most critical during transitions of care, such as at discharge or when a patient moves between different levels of care, or receives care from VA and community providers. These transitions increase the risk of medication-related errors due to miscommunication. To reduce this risk, healthcare teams must ensure that, at each transition point, the patient or caregiver clearly understands the treatment plan.”
