Ashy dermatoses, Erythema dyschromicum perstans: overview, features, histology, management

History
M=F, usually young adults
Mainly in those of intermediate skin colour (mainly South/Central America, India)
Usually asymptomatic, slight pruritus may be present

Exam findings
Numerous macules of varying shades of ashen/slate-grey to brown-blue colour
May have a thin, raised, erythematous border in early stages – may be absent later or evolve into hypopigmented border (darker skin types)
May have macules of hypomelanosis or hypermelanosis against the greyish background
Vary in size and shape
Mainly face, neck, trunk, proximal part of the arms, axillae, submammary

Histology
Biopsy
Active border shows lichenoid changes (vacuolar degeneration of basal cells, occasional colloid bodies), epidermal melanin, pigmentary incontinence and melanophages, dermal vessels surrounded by lymphocytes and histiocytes
Inactive: no vacuolisation of basal cell layer, diminished dermal infiltrate, increased dermal melanophages
