Closed Iniencephaly of fetus | A Rare birth defect | Complex neural tube defect

Iniencephaly is a rare and complex neural tube defect (NTD) that occurs very early in pregnancy. It is characterized by extreme retroflexion of the head, shortened trunk, marked lordosis in the cervicothoracic vertebrae, absence or partial absence of the occipital squama, abnormal fusion of vertebrae, closed vertebral arches, and formation of an encephalocele (for iniencephaly apertus). The malformations of iniencephaly occur in the early pregnancy prior to the closure of the cephalic neural fold at 24 days of gestational age.

In most cases, the baby dies during pregnancy or shortly after birth. Iniencephaly of both types carries a lethal prognosis, sometimes even ending in spontaneous abortion or stillborns. The defects in iniencephaly are diagnosed through prenatal ultrasound and can be further examined through CT and MRI. Once established, decisions can be made with regard to terminating the pregnancy or providing a plan of adequate postnatal care.

Differential diagnosis is important since many of the characteristics of iniencephaly, such as congenital retroflexion of the spine and fusion of the cervical vertebrae, are shared with other disorders.
