This is a tougher one…

📌Buccal trough with 703 fissure bur and section the roots buccolingually

📌Had to use a purchase point to get the distal root out

📌I tried the purchase point on the mesial root but it only moved it a little so I sectioned it and removed both segments

📌4mm inferior alveolar nerve exposure under distal root and the nerve is intact

📌Irrigated well and placed a PRF Plug which will release back growth factors locally over 2 weeks

📌Closure with 4-0 chromic
