Kuchisake-onna is a malevolent figure in Japanese urban legends and folklore. According to the legend, she is the vengeful spirit, or onryō, of a woman who was mutilated and has returned to seek revenge on the world. Her name, Kuchisake-onna, translates to "Slit-Mouthed Woman" in English.

The story goes that Kuchisake-onna was once a beautiful woman who was married to a samurai. However, her husband became jealous and accused her of infidelity. In a fit of rage, he attacked her, slashing her mouth from ear to ear, giving her a grotesque, permanent smile.

After her death, Kuchisake-onna's spirit returned as a vengeful ghost. She is often depicted wearing a surgical mask to hide her disfigured mouth. In some versions of the legend, she carries a sharp object, such as a pair of scissors or a knife.

Kuchisake-onna is known for her eerie encounters with unsuspecting victims. She is said to appear to people, usually at night, wearing her mask and asking them a question. She will ask, "Am I pretty?" or "Do you think I'm beautiful?" If the person answers "no," she will kill them on the spot. If they answer "yes," she will remove her mask, revealing her horrifying, slit-mouthed face, and ask the question again. If the person answers "no" the second time, she will kill them. If they answer "yes" again, she will follow them home and kill them later.
