How to Schedule an Appointment at the U.S. Embassy
Navigate to https://cacms.state.gov/s/new-appointment
Click Israel, West Bank and Gaza on the dropdown menu.
Click "I'm not a robot"
Click "Next"
Click "The U.S. Embassy offices in Jerusalem and Tel Aviv Covering, Israel_West_Bank_and_Gaza"
Click "Next"

Click on the type of appointment that you need.

A. Jerusalem - Passport renewal (DS-11)
14 David Flusser St., Arnona, Jerusalem-------*Minor Passport Renewal, * First Adult Passport, *Any other Application requiring a DS-11

These emergency appointments are for U.S. citizens who need to apply for a passport using form DS-11. The applicant will receive a purple EMERGENCY PASSPORT ONLY. You can replace the limited-validity, emergency passport for free within one year of its issuance by mail, whether you are in the United States or in Israel, the West Bank, or Gaza, or by drop box in Jerusalem.
