Today, I want to talk about how to approach the patient who presents with common cold like symptoms.

It’s definitely the season for the common cold! But, you also need to consider other possible etiologies, and deciding whether giving antibiotics, or a getting chest x-ray for your patient can be difficult at times.

The first thing that needs to be done, is eliciting the patients past medical history.

Are they a smoker?
Are they immunocompromised?
Do they have Eustachian tube dysfunction?
Is there a history of frequent sinus infections?
Is the patient homeless (this predisposes the patient to many other diseases)?

In today’s lecture, we’ll discuss:

The questions you need to ask your patient
What “bad symptoms” need to be ruled out that might require further workup
Rapid fire overview of influenza, acute bronchitis, acute sinusitis, pneumonia, strep throat
How to calculate the CURB-65 score
The Centor Criteria
How to manage their symptoms if the etiology is viral


Here are the URI history questions discussed in today's lecture:
1) Whats going on?
2) How long have symptoms been present?
3) Any sore throat, ear pain, sinus pain/pressure?
4) Bad symtoms:
SOB, wheezing, chest tightness pleuritic chest pain?
Fever weight loss, night sweats, hemoptysis?
