After getting his revenge following the events of Twisted Metal: Black, Mr. Grimm is now a free man after spending (supposedly) 30 years in solitary.

Though, not even Grimm is sure if really exists or not, as the lack of medication has caused him delusions like no other; often seeing an apparition of his dead friend who he was forced to eat back in Vietnam.

After killing that advisor, Mr. Grimm had realized that over the years he'd developed a special craving...

His taste taste for human flesh has not abated. His sanity has not returned. He now spends his nights racing through the streets of Harbor City, searching for his next meal while speaking to the rotting skull of his dead friend that he wears as a helmet.

In recent years, he may have began to realize his true destiny. He began to think he may actually be the Grim Reaper...

But deep down, he knew it wasn't real. It was all just a delusion; something that he told himself when the hunger became too much for him to handle.

Until one night, Calypso reemerged from the shadows, offering him the chance of a lifetime to make this dream of his finally turn into reality.

Although he told himself that he wasn't going to do this again, the hunger didn't care what he promised. He agrees to play.

This story follows Mr. Grimm entering the Twisted Metal contest once again. This time around, we dive deeper into the haunted psyche of a man forever scarred by his past.

Still tormented by the horrors of war and a dark hunger born in the jungles of Vietnam, Grimm has wandered through the chaotic streets of Harbor City, devouring human flesh to fill the void within.

Along the way, he’s haunted by visions of his fallen friend Benny and the dark experiments he endured in Blackfield Asylum. Every fight, every mile, draws him closer to what he believes is his calling.

This story blends ideas from the 1990 horror film, "Jacob's Ladder", as well as mixing elements of psychological horror and a supernatural transformation like in Ghost Rider.
