In this case, the phaco probe punctures through the central lens nucleus as well as the peripheral lens equator. The posterior capsule is further ruptured when the resident surgeon performs a cracking technique which places stress on the capsular bag. Next, nuclear pieces fall into the vitreous cavity. How will this case be recovered? How can we achieve a good outcome for the patient?
