Care plans involving social prescriptions are set to play a bigger role as Singapore moves towards preventive care. But for health and social care workers to recommend appropriate community activities for patients to live and age well, they first need to have the right skills. A new arm under SingHealth hopes to train 600 of them every year. Across SingHealth, wellbeing coordinators help to identify patients who face social risk factors and link them up with the right social prescriptions. Nearly one thousand patients have benefited so far.
