We present a case of a 37-year-old man diagnosed with seronegative immune-mediated necrotizing myopathy (IMNM) after presenting with dysphagia, elevated creatine kinase, and transaminitis. Despite initial worsening, his condition significantly improved with high-dose IV methylprednisolone, confirming the diagnosis through a muscle biopsy and leading to a successful recovery with an oral prednisone taper.
