Role of Fractional Flow Reserve (FFR) RFR, IFR and FFR CT in assessment of blocks in the coronary arteries of the heart.

All blocks in the heart may not need stents especially if they do not produce restriction to blood flow to the heart muscle. Angiography is only a visual estimate. A block may appear borderline on Angiography but may not affect blood flow. Stenting of such blocks may not be useful and can be detrimental too.

FFR RFR IFR helps in assessing if blood flow is affected because of a block. FFR is the current gold standard test for this purpose. Non invasive tests like Stress MPI and evolving CT FFR are alternatives.

FFR involves introduction of a wire with a sensor into the coronary artery and administration of a medicine called adenosine which is a vasodilator. If the FFR value is less than 0.8 stent will be useful where as if it is more than 0.8 stent will not ne needed and Medical management is preferred.

FFR has helped in avoiding stent implantation in numerous patients.
