Mafia is set during the 1930s in the fictional American city of Lost Heaven, with a countryside encompassing 12 square kilometers in the game. The city incorporates parts of San Francisco and Chicago of the same time period.

The game traces the rise and fall of Tommy Angelo, a fictional mafioso in the 1930s, once a local taxi driver, is strong-armed into helping two mobsters working for crime boss, Don Salieri, escape from an ambush planned by members of the Morello family, and is compensated for his trouble with a promise of assistance if he requires it."
