When there is a vertebral fracture we have to analyse why is the fracture, whether it is because of the basic pathology in the bone itself or is it because of the trauma that is produced because the vertebral fracture can happen because of slight trauma like if somebody slips or somebody suddenly sits and develops a back pain and when you go for an x-ray or an MRI you see a fracture. So these usually occur because of the pathological reason. Osteoporosis is nothing but weakness of the bone usually when there is a demineralisation or when the bone does not get mineralised properly or it doesn’t develop enough strength and usually occurs with Vitamin D deficiency or occurs during later part of life, that is in old age. Therefore in such cases when then bone itself is osteoporotic or when the bone is very much weak even trivial injuries can produce fracture. Here what we have to focus is on the building up of the bone. So to recover from osteoporosis, that may involve giving sufficient amount of calcium for months or years or sufficient amount of Vitamin D which is also essential for the absorption of sufficient amount of calcium and we have to give chemical substance like bisphosphonates, which are essential for mineralisation or strengthening of the bones. sometimes we have to give parathormone when it is very severe osteoporosis, in injection forms we have to give. These are the medical line of treatment for osteoporosis or the fracture of the bone suppose the fracture is very severe and if it is pressing upon the spinal cords, then depending upon the symptoms if he has the neurological symptoms sometimes the fracture can be associated with total paralysis or partial paralysis of the lower part of the body or suppose if it happens in the cervical spine then it can involve all the four limbs. In such cases then we have to operate or we have to release the pressure on the nerve. That involves surgical treatment. Otherwise majority of the cases we can treat them without surgery or just building up of the bone, osteoporosis treatment.
